The Michigan Conference of Teamsters Welfare Fund’s (MCTWF) website is owned and operated by the Michigan Conference of Teamsters Welfare Fund. These terms and conditions of use, along with other legal notices, disclaimers, and requirements that may be contained on this website, will apply to and govern your use of this website. By accessing or using any aspect of MCTWF’s website, you are acknowledging your consent to the following terms and conditions. MCTWF reserves the right to change these terms and conditions from time to time at its discretion. Registration and Submission to Michigan Conference of Teamsters Welfare Fund
Certain areas of the MCTWF website are accessible only to registered users. To become a registered user, you must be a MCTWF participant or covered dependent. Children below 18 years of age wishing to register, must first contact MCTWF’s Customer Communication department for authorization. In order to access the registered-user-only areas of MCTWF’s website, you will need to establish and maintain a personal username and password. You acknowledge that the use of username and password is an adequate form of security and you will not reveal this information to anyone. You are solely responsible for (1) authorizing, monitoring, controlling access to and maintaining the strict confidentiality of your username and password, (2) not allowing another person to use your username or password, (3) any charges or damages that may be incurred as a result of your neglect to maintain the strict confidentiality of your username and password, and (4) promptly informing MCTWF in writing of any need to deactivate a username due to security concerns. MCTWF is not liable for any harm related to the theft of usernames or passwords, disclosure of usernames or passwords, or your authorization to allow another person or entity to access and use MCTWF’s website using your username or password. You shall immediately notify MCTWF of any unauthorized use of your username or password and any breach of confidentiality. Until MCTWF receives this notification from you, you will be held liable for any harm ensuing from the use of your username on MCTWF’s website.
Submissions to Michigan Conference of Teamsters Welfare Fund’s Website
If any user of MCTWF’s website makes any submission by way of comment, suggestion, or question which contains any ideas, concepts or techniques, the user’s submission, and any information submitted with it, shall be considered non-confidential and shall be considered the sole and exclusive property of MCTWF. MCTWF shall have no obligation of any kind (such as compensation) with respect to the submission or any materials that accompany it. MCTWF shall be free to use or transmit to others any ideas, concepts, or techniques contained in the user’s submission and accompanying materials for any purpose whatsoever.

Customer Services Applications
The customer services web-based applications are exclusively provided to MCTWF customers. Use of these applications is subject to the terms of agreements signed with MCTWF.
Severability
If any provision of these terms and conditions is found to be unenforceable or invalid under any applicable law, such unenforceability or invalidity shall not render these terms and conditions unenforceable or invalid as a whole, and such provision shall be construed so as to limit any provision so as to make it enforceable or valid within the requirements of any applicable law, and in the event such term or provision cannot be so limited, these terms and conditions shall be construed to omit such invalid or unenforceable provisions.
Jurisdiction
MCTWF’s website is operated as a service to the participants and beneficiaries of MCTWF in the United States of America, its territories and Canada, and should be used only by residents of the United States of America and Canada. These terms and conditions will be governed by and construed in accordance with the laws of the State of Michigan, without regard to its conflicts of law provisions. If any of these terms and conditions is determined to be void, unlawful, or unenforceable, the validity and enforceability of the remaining terms and conditions shall not thereby be affected or impaired.
