Omada for Prediabetes and Weight Management

Teladoc Health Diabetes Management
The Teladoc Health Diabetes Management program is offered at no cost to members, age 13 and up, with diabetes and medical coverage through the MCTWF Actives Plan or MCTWF Retirees Plan. The program includes a free glucometer, unlimited strips and lancets, digital monitoring resources, and more. Caremark Performance Drugs

Advanced Control Specialty Formulary
The CVS Caremark Advanced Control Specialty Formulary is a guide within select therapeutic categories for plan members and healthcare providers. Your benefit plan provides you with prescription drug coverage that is administered by CVS Caremark. Ask your doctor to consider prescribing, when medically appropriate, a preferred medicine from this list.

Specialty Drug Benefit Alignment
Link to the Specialty Drug Benefit Alignment Program for the current list of affected specialty medications. The CVS/caremark Specialty Drug Benefit Alignment Program (also referred to as Specialty Medical Carve-Out Program) allows for select oral and injectable specialty medications which are provided and/or administered at a physician’s office. For your convenience, the benefit was carved out of the MCTWF Plans’ medical benefit and covered instead under the MCTWF Plans’ pharmacy benefit through CVS/caremark Specialty Pharmacy. Telehealth Consultations
Link to MDLIVE for telehealth consultations with Board certified doctors and licensed therapists by phone, or secure video. This service is available to all eligible medical benefit participants and their covered beneficiaries.
