Use the links below to obtain provider network information for Blue Cross Blue Shield, Caremark, MultiPlan, Delta Dental, DenteMax, and DeltaVision, subject to network limitations stated in your Summary Plan Description.
Blue Cross Blue Shield
For Michigan Physician Searches -
Click on  , then click on "Find a Participating Provider". Next click on "physician search". In "Step 1", under the "Choose a plan" selection, click on "MCTWF PPO". If you want to narrow your search complete the "Step 2" selections then click on the search button to complete your search, otherwise just click on the search button.
For Michigan Hospital and Facilities Searches -
Click on , then click on "Find a Participating Provider". Next click on "hospital and facilities search" and click on the "Facility type" you are searching for, then click on "Community Blue/Blue Preferred PPO" in the "Plan type" selection and click on the "Facility County" you are searching for. Then click on the "search" button to complete your search.
For Non-Michigan Physician, Hospital and Facilities Searches -
Click on  , then click on "Member", enter TMI as your "Identification Prefix" and click on the "continue" button .Next, complete the "Search Center" and "Search Radius" selections then click on the "search by specialty" button. In the "Provider Types" selection click on the type you are searching for, and in the "Specialties" selection click on the specialty you are searching for, then click on the "continue" button to complete your search.
Caremark

Link to Retail 90 for available Caremark participating Retail 90 pharmacies. To locate a participating pharmacy, enter the pharmacy name and select the city and state, or, if you would like to search for any participating pharmacy, select only the city and state.
