Q: What happens to my benefits if I switch back and forth between DeltaPremier network dentists and dentists who are not in the network? A: You may switch between network and non-network dental providers whenever you choose. Each claim will be paid according to which provider you have chosen. The network provider will receive the network benefits and the non- network claim will be paid according to the non-network benefit plan. The Explanation of Benefits form (EOB), provided to you with each claim processed, will indicate how the benefit schedule has been applied and what amounts you are responsible for. Be aware that the per person annual maximum amount is accumulated between both network and non-network services. Comments
